StrikerDirect collects the information you provide through our setup form — such as your practice name, contact name, role, email, phone number, state, practice type, and message — solely to respond to your inquiry and set up ordering with Striker Pharmacy.

We do not sell your information. We share it only with Striker Pharmacy LLC and service providers necessary to onboard your practice. Please do not submit protected health information through this form.
